In 2023, Community & Home Independence Partners began as a mobile outpatient private practice, delivering OT services in home and community settings where patients experience the most difficulties. We believe that occupational therapy is an under-utilized service which can be critical in supporting people with neurological conditions to slow progression, manage symptoms, and maximize independence to improve quality of life. At Community & Home Independence Partners, we focus on home safety, community engagement, and improving quality of life and opportunities for daily engagement via skill development, use of tools, and modifications to environment and task. Our services, including LSVT-BIG, are covered by Medicare, and we are able to bill as an out-of-network provider for most insurances. Owner, Jillian Bucolo, is certified in LSVT-BIG, has earned an Execeutive Certificate in Home Modification (ECHM), and has extensive experience working with people in their living environment to maximize safety.
